A cycle shop I do some work for uses Ross Pistons exclusively so I called and they said they could make a set of forged 96mm with dome for my 71 crank with pins and rings for under $450.
Their stock line I have used up to 104's and are really, really nice with lightweight circlip retainers and pin oilers built in. I haven't done any special orders from them before though.
