Dan,
Fat Performance does the 5th stud. I can't
recall who does a base plate under the cylinder for a six stud arrangement.

Alfred talks about the six stud plate setup that is done in Europe on his message board: http://www.insidetheweb.com/mbs.cgi/mb1084902
It requires both CNC machined plates and custom studs.
